Subdivision Applications
Application Instructions
The following application forms shall be used when applying for a new subdivision or to make amendments to an existing subdivision in the Town of Wells. When submitting your application form, please provide all form pages. Omission of application form pages will not be considered a complete submission.
Application Fee and Escrow Deposits
Pre-Application Forms do not require an application fee. All other Application Forms require an application fee to be submitted. See the Fee Schedule. Application fees shall be paid by check, submitted to the Planning Office, and made payable to the "Town of Wells." Application fees are non-refundable.
All applications require the submission of escrow. Pre-Applications require a minimum deposit of $40 to cover abutter notification costs. All other Applications require a minimum deposit of $400 to cover abutter notification and public hearing costs. If additional escrow is required during the review of the application the applicant shall be responsible for providing those funds upon request. Any escrow funds remaining in the application escrow account shall be refunded to the depositor once it is determined by the Planning Office that all necessary expenses have been paid for that project.
Subdivision Pre-application
This form should be accompanied with a sketch plan outlining your concept for the new or amended subdivision. Pre-applications / sketch plans are reviewed and considered for free with the exception of postage and copying costs incurred by the Planning Office. A $40 fee is recommended upon submission of a subdivision pre-application. Preliminary Subdivision Application
This form should be submitted within 6 months of your pre-application / sketch plan site walk; if more than 6 months passes another site walk may be required by the Planning Board. Preliminary Subdivision Application Forms are necessary for all major subdivisions (subdivision containing more than 4 lots or dwelling units or proposes a street). If you are proposing an amendment that meets the definition of a major subdivision, the Preliminary Subdivision Application Form is also needed. An application fee and escrow deposit is required upon the submission of a Preliminary Subdivision Application. Final Subdivision Application
This form should be submitted within 6 months of preliminary subdivision approval; if more than 6 months passes your preliminary approval is voided and re-submission and review of the Preliminary Subdivision Application is required. Another application fee is also assessed. If you are proposing an amendment that proposes no new lots or dwellings or streets the Final Subdivision Application Form should be submitted. A fee and escrow deposit is required upon the submission of a Final Subdivision Amendment Application. Minor Subdivision Application
This form should be submitted within 6 months of your pre-application / sketch plan site walk; if more than 6 months passes another site walk may be required by the Planning Board. Minor Subdivision Application Forms are necessary for all minor subdivisions (subdivision containing no more than 4 lots or dwelling units and no street is proposed). If you are proposing an amendment that meets the definition of a Minor Subdivision (1 to 4 new / amended lots / dwelling units proposed with no street), this form is also needed. Site Plan Applications

The following Application Forms shall be used when applying for a new use that requires Site Plan approval or to make amendments to an existing Site Plan in the Town of Wells. When submitting your application form, please provide ALL form pages. Omission of application form pages will not be considered a complete submission.

Site Plan Pre-application
This form is used when applying for a any site plan review and you are looking for initial feed back on a sketch plan, are unsure of the exact uses proposed or layout of the project, or are anticipating the need for the Reviewing Authority to conduct a site walk of the property. No application fee is required with the submission of a Site Plan Pre-application. Abutters are notified of a Pre-application submission and a small escrow deposit of $40.00 is recommended to cover initial abutter notification costs. The submission of a Site Plan Pre-application for properties that have current site plan approval is not required. Site Plan Application
This form is used when applying for formal site plan review. If a pre-application was submitted previously and a site walk was conducted, the Site Plan Application must be submitted within 6 months of the site walk, or else the Reviewing Authority may require another site walk. Site Walks are not conducted during the months of January, February or March, during inclement weather, or when the ground is covered in snow, so please take this into consideration for your submissions. This form is also submitted when applying for an amendment to a previously approved Site Plan. A site plan application fee and escrow deposit is required upon the submission of a Site Plan Application. Abutters are notified of a Site Plan Application submission. A Public Hearing may also be scheduled during the review of this application.
